Your health information is some of the most personal information there is. This page explains what we collect through this website and our practice, how we use it, and the choices you have.

What this policy covers
This policy applies to Love Your Health Natural Medicine ("we", "us", "our"), a naturopathic medical practice located at 123 E Baseline Rd, Suite D207 #1-3, Tempe, AZ 85283, and to this website.
It covers two different kinds of information, and they are governed by different rules. Information you submit through this website, such as a contact form, is covered by this policy. Information created when you become a patient, such as your chart, lab results, and visit notes, is protected health information and is governed by HIPAA and our Notice of Privacy Practices, which we provide to you at your first visit.

Information we collect
Through this website, we collect only what you choose to send us:
- Contact form. Your name, email address, phone number, practitioner preference, and whatever you write in the message field. This form is provided by HighLevel, our scheduling and contact system, so submissions go to them rather than to this website.
- Server logs. Our hosting provider records standard technical information such as IP address, browser type, and the pages requested. This is used to keep the site running and secure.
Please do not include detailed medical information in the website contact form. It is a general enquiry form, not a secure patient portal. Once you are a patient, use the Charm patient portal to message your care team about anything clinical.
As a patient, we also collect the information needed to care for you: health history, medications and supplements, lab and imaging results, insurance details where relevant to labs and imaging, and payment information.

How we use your information
- To respond to your enquiry and, if you are requesting an appointment, to contact you and confirm a time.
- To provide, coordinate, and document your care.
- To order labs and imaging, and to bill insurance for labs and imaging where applicable.
- To collect payment for visits and services.
- To operate, secure, and improve this website.
- To meet our legal, licensing, and record-keeping obligations.
We do not sell your personal information, and we do not sell or rent your email address.

Health information and HIPAA
Your medical record is handled separately from this website.
As a healthcare provider, we are a covered entity under the Health Insurance Portability and Accountability Act. Your protected health information is handled under HIPAA and Arizona law, and is described in full in our Notice of Privacy Practices, which you receive at your first visit and can request from the front desk at any time.
We use Charm as our electronic medical record and patient portal. Intake forms, consent forms, lab results, and clinical messages are handled inside Charm rather than by email, and Charm operates under a business associate agreement with us.

Calls, texts, and email
You can call, text, or email our office. If you text or email us, please keep in mind that standard text messaging and standard email are not encrypted and are not fully secure channels. Choosing to contact us that way is your decision, and we will reply in kind unless you tell us otherwise.
For anything clinical, the Charm patient portal is the appropriate channel. Message rates from your mobile carrier may apply to texts. You can ask us to stop texting you at any time.

How we protect your information
We use administrative, physical, and technical safeguards to protect the information in our care, including access controls on our medical record system, encrypted connections on this website, and staff training on privacy obligations.
No method of transmission or storage is completely secure, so we cannot guarantee absolute security. If a breach affecting your protected health information occurs, we will notify you as required by law.

Your choices and rights
- Access and copies. You may request a copy of your medical record. Your records are also available to you through the Charm mPHR app.
- Corrections. You may ask us to amend information in your record that you believe is inaccurate.
- Restrictions. You may request a restriction on how we use or disclose your information. We will tell you if we cannot agree to it.
- Accounting of disclosures. You may request a list of certain disclosures we have made.
- Complaints. You may complain to us, or to the U.S. Department of Health and Human Services Office for Civil Rights. We will never retaliate against you for filing a complaint.

Children's privacy
This website is not directed at children and we do not knowingly collect information from children through it. We do treat minor patients, subject to each doctor's age policy and to Arizona law on parental consent, and their records are handled with the same protections as any other patient record.

Changes to this policy
We may update this policy as our practice or this website changes. When we do, we will revise the "last updated" date at the top of this page. Material changes to how we handle protected health information will also be reflected in our Notice of Privacy Practices.
