Reading this website does not create a doctor-patient relationship. That relationship begins only when you become an established patient of the practice.

Appointments cannot be self-scheduled on this website. You may request a time through our contact form, request a free discovery call, or call or text the office. We will contact you to confirm.
A $50 booking fee is collected when your appointment is scheduled and is applied toward the cost of your visit. Submitting a request does not guarantee a specific appointment time until we confirm it with you.
We require at least 24 hours notice to cancel or reschedule. Late cancellations and no-shows incur a $50 fee. If a visit runs beyond its scheduled length, the additional time is billed at $40 per 15-minute increment.
We hold these terms because a missed appointment is time that could have gone to another patient. If something genuinely unavoidable happens, talk to us.
Our doctors are licensed in Arizona, so virtual visits are available only to patients physically located in Arizona at the time of the appointment. You are responsible for having a private space and a working internet connection.
Some things cannot be assessed remotely. If your doctor determines that you need to be seen in person, we will tell you and arrange it.
Any prescription, supplement, injection, or peptide therapy is provided at the clinical discretion of your doctor, after evaluation and where appropriate testing, and only where it is suitable for you. We do not prescribe on request alone.
Follow the dosing and monitoring instructions you are given, and tell us about every medication and supplement you take, including anything obtained elsewhere.
Outcomes vary from person to person. Any timelines, case descriptions, testimonials, or statistics on this site describe individual experiences or published research and are not a promise of any particular result for you.
You may call, text, or email the office. Standard text and email are not secure channels, and choosing to use them is your decision. Clinical questions should go through the Charm patient portal.
Message and data rates from your carrier may apply. You can ask us to stop contacting you by text at any time.
To the fullest extent permitted by Arizona law, we are not liable for indirect or consequential losses arising from your use of this website or your reliance on its general information. Nothing in these terms limits any liability that cannot be limited by law, including liability relating to the professional care we provide.
How we handle your information is described in our Privacy Policy. Your medical record is additionally protected under HIPAA and our Notice of Privacy Practices, which you receive at your first visit.
These terms are governed by the laws of the State of Arizona, and any dispute will be handled in the courts of Maricopa County, Arizona.
We may update these terms from time to time. The "last updated" date at the top of this page tells you when they last changed. Continuing to use the site after a change means you accept the updated terms.
